Every request for policy is to be based on a proposal form to be submitted by you. This form seeks basic information regarding yourself. Information sought in the proposal should be given in all respects, please also give your contact number and E-mail ID (if any).
The prospectus gives you details of the health insurance policy. Please read it fully and carefully before you complete the proposal form.
Insured Person details form seeks important information on each of the persons to be covered under the policy. Please complete the insured person details form in respect of each family member you wish to cover under the health insurance policy and submit an ID like passport, voters ID card, Pan card, Driving Licence.
Stamp size photograph of each insured person is to be submitted along with the insured person details.
Please declare all pre-existing illness / injury / disease / conditions for each insured person at the time of proposal. No material information should be kept hidden from an insurer.
A copy of the duly filled proposal should be signed only by you as the person interested along with the date on which you sign.

A copy of the proposal filled up and signed by you will be given to you along with the policy copy within 30 days of the issuance of the policy.
