CLAIMs DOCUMENT REQUIRED

• Claim form duly completed
• First Information Report lodged with the police in case of personal accident /loss of baggage
• Death certificate in case of death
• Doctor’s certificate in case of disability
• Documents to substantiate the cause and quantum of loss
• Any other document required by the policy issuing office in proof of loss/quantum

CLAIM PROCEDURE

• Intimate the policy issuing office or the nearest office of the company.
• Claim form and other documents as required by the Insurance Company should be furnished.
